چکیده
pedunculated polyp A 46-year-old woman was admitted to a local medical center with abdominal pain and discomfort, which she had experienced for 6 months. Her medical history was unremarkable. Esophagogastroduodenoscopy (EGD) showed a huge pedunculated polyp located in the stomach body. The patient was referred to our hospital for endoscopic submucosal dissection (ESD) with the working diagnosis of hyperplastic polyp. On repeat EGD, an 8-cm pedunculated and lobulated polyp with broad base was observed in the posterior wall of the distal gastric body (●" Fig.1). Laparoscopic distal gastrectomy was performed because the polyp was too large for ESD. On gross examination, the tumor, which measured 7cm, was ill-defined with a fibro-fatty appearance and mixed elastic-soft and solid consistency. Microscopically, the tumor consisted of mature adipocytes, with variation in cell size and scattered, bizarre, hyperchromatic stromal cells in a fibrillary and collagenous background (●" Fig.2). The tumor cells showed positive nuclei staining for MDM2 (●" Fig.3) and CDK4 (●" Fig.4). The final histopathological diagnosis was that of a well-differentiated liposarcoma. The patient was considered for adjuvant treatment because the resection margin was positive for liposarcoma. Liposarcoma accounts for 15%–20% of all sarcomas and usually affects the extremities and retroperitoneum [1]. However, gastric liposarcoma is extremely rare, with only 13 cases reported in the literature. The recently updated World Health Organization classification recognizes four major subtypes of liposarcoma: atypical lipomatous tumor/well-differentiated, de-differentiated, myxoid, and pleomorphic liposarcoma [2,3]. Although onethird of well-differentiated liposarcomas show local recurrence, metastasis is virtually never seen unless de-differentiation occurs [3]. The most important prognostic factor is anatomical location [1,4]. Overall mortality ranges from 0% for atypical lipomatous tumor of the extremities to nearly 80% for tumors occurring in the visceral sites and retroperitoneum [2,3].

A RARE CASE OF GASTRIC LIPOMA
A middle age woman was hospitalized for investigation of a mobile filling defect in the stomach with exacerbation of 3-year gastric symptoms. Gastroscopy revealed a huge pedunculated polyp. Histologic report of the mass was gastric antral lipoma. Since the patient refused surgery, cauterization polypectomy was performed without any complication.
متن کاملA Pedunculated Hamartomatous Polyp of the Palatine Tonsil
Hamartomatous polyps of the palatine tonsil are very rare. They have been variously termed as a lymphangiectatic fibrous polyp, lipoma, pedunculated tonsil etc. in the English literature. We present here a case of hamartomatous polyp of the tonsil occurring in a 32-year-old male who presented with bilateral enlarged palatine tonsils with recurrent tonsillitis. Surgical excision showed two pa...
متن کاملSymptomatic gastric leiomyoma mimicking giant gastric polyp: endoscopic diagnosis and removal.
Gastric leiomyoma is an uncommonly found benign submucosal neoplasm which may cause hemorrhage in rare cases. A 67-years-old patient was admitted to our attention due to two episodes of hematemesis and melena occurred in the previous two days. Esophagogastroduodenoscopy showed a giant semi-pedunculated gastric polyp (diameter of 4 centimeters) located at the distal body. A large ulcer with a cl...
متن کاملUnderwater polypectomy: successful endoscopic resection of a large gastric pedunculated polyp
Polypectomy is the standard therapy for gastrointestinal pedunculated polyps [1]. However, achieving effective countertraction to snare the stalk of the polyp located in the direction of gravity can sometimes be difficult [2]. Herein, we present a case of a large gastric polyp located on the greater curvature of the gastric body that was resected using the “water immersion” technique in order t...
متن کاملA Case of Lobulated and Pedunculated Duodenal Hyperplastic Polyp Treated with Snare Polypectomy
We report herein the case of a lobulated and pedunculated hyperplastic polyp in the third portion of the duodenum causing anemia and occult blood in stools, which was detected by capsule endoscopy (CE) and treated with snare polypectomy. A 71-year-old man was referred to our hospital because of anemia and occult blood in stools. Three months earlier, he had been admitted to another hospital bec...
